Elwood O'Neil is a member and the leader of the O'Neil Brothers, a family of rednecks who produce meth in Blaine County, and notably Trevor's main rivals in the drug trade.

The O'Neils were raised in a ranch in Grapeseed, which they still use as a home and meth lab. Most of the brothers are killed by Trevor after he finds out the O'Neils are doing business with the Cheng family. Elwood, Walton and Wynn survive this attack and set out to avenge their brothers, but are killed near the Raton Canyon by Franklin, Michael and Trevor.
